Once the palatal suture fuses in the mid-to-late teens, a conventional expander can no longer separate it through the teeth alone. MSE is considered once the palatal suture has fused — typically from the mid-to-late teens onward — which is exactly when a conventional expander can no longer achieve bony widening through the teeth alone. Because the four mini-implants anchor directly into the palatal bone rather than the teeth, many adults who were previously told expansion would require jaw surgery are candidates for MSE instead; suitability is confirmed on a CBCT scan, not from a clinical exam alone.يُنظَر في MSE بعد التحام الدرز الحنكي — عادةً من منتصف إلى أواخر سنوات المراهقة فصاعداً — وهي بالضبط اللحظة التي لم يعد فيها الموسّع التقليدي قادراً على تحقيق توسيع عظمي عبر الأسنان وحدها. ولأن البراغي المصغرة الأربعة تُثبَّت مباشرة في عظم الحنك وليس الأسنان، فإن كثيراً من البالغين الذين قيل لهم سابقاً إن التوسيع يتطلب جراحة فك أصبحوا مرشحين لـMSE بدلاً من ذلك؛ ويُؤكَّد الترشح بمسح CBCT وليس بالفحص السريري وحده.
