What information this website collects

The only information this website asks you for is what you type into the consultation enquiry form. That form contains the following fields:

• Required: full name, phone number, email address, and preferred clinic.
• Optional: date of birth, country, city, how you heard about the practice, your main concern, and a free-text message.

The “main concern” menu and the free-text message are about your health — for example jaw pain, snoring or breathing. Please send only what you are comfortable sending by email, and do not use the form for anything urgent.

This website does not run any analytics, advertising or visitor-tracking script. It stores one preference in your browser to remember whether you are reading in English or Arabic. This is explained in full in the Cookie Policy.

How your enquiry reaches the practice

When you press submit, the form is sent to a third-party form-delivery service, FormSubmit (formsubmit.co), which forwards the contents to the practice mailbox at hello@drpachaorthodontics.com. Your enquiry therefore passes through a company other than the practice in order to be delivered. It is then held in the practice’s email system in the same way as any other message you might send.

[TO BE CONFIRMED] Whether a written data processing agreement is in place with FormSubmit, where that service stores messages, and for how long, has not been established. This must be confirmed before this notice is published.

Why the practice uses this information

• To arrange a consultation at the clinic you selected.
• To keep a record of the correspondence between you and the practice.

Your details are not sold, and the practice does not use them to send marketing you did not ask for.

How long information is kept
